Hmm from what I know this is the only cover for the original 7".
However I just saw that a comp. reissue was released with this single included.. maybe it's that one you are thinking of?

I remember buying this at Bullit in Eindhoven for 10 guilders (4.50 euro !!)
They had 4 copies there if I remember correctly, if only I knew than what I know now :)
